Community Mental Health Nurse

Assignments:
Work as a Residential Nurse Case Manager and may function as a supervising nurse in Saratoga/Washington Counties. Duties to include but not limited to:
o Supervises overall individuals' health care in assigned homes which can include medically fragile homes, working with House Leaders to implement optimal care. Assess individuals on an ongoing basis, identifying medical/health needs and updating discharge transition. Interact with Primary, medical specialists, various clinical staff as necessary o Advocate for individuals' health care needs o Working with the residence House Leader to coordinate appointments for primary visits, consultant services and required tasks. o Provides oversight to individuals when hospitalized including the discharge transition. Completes follow-up including documentation for individuals post ER/hospitalization. o Provide nursing component to the Interdisciplinary Team. o Ensures that medications and treatments are administered according to approved policies and regulations. Provides ongoing oversight to staff administering medications. o Provides training and oversight to staff in the area of medication administration, OSHA (Blood Borne and TB); and individual specific training needs o May also function to Supervise the health care of individuals living in family care by working with the family care provider, to implement optimal care. o Attends/completes on-line mandated DDSO training, as well as discipline specific training o Complete special projects/assignments as assigned. o Conduct investigations as assigned by supervisor.

Salary Range:
From $75323 to $92429 Annually

Eligible List:
Candidates must be reachable for appointment on Current Civil Service Eligible List for this title. OR Transfer:
Candidates must have one year of permanent service in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1 of Civil Service Law. OR Temporary:
Community Mental Health Nurse is a continuous recruitment examination. Candidates who are not currently on the eligible list may apply for this position and if appointed, will receive a temporary appointment. Candidates who receive a temporary appointment must receive a score that is reachable on the resulting eligible list before they can be permanently appointed. To qualify, candidates must be licensed and currently registered to practice nursing in New York State, or possess a limited permit to practice nursing in New York State. (You must obtain your New York State license within one year of the date of appointment or action will be taken to remove you from the position.) AND have a bachelor's or higher degree in one of the areas listed below ; AND two years of professional clinical nursing experience, including at least one year as described below ; OR have 30 college credits in the areas listed below IN ADDITION TO an associate's degree in nursing or completion of a three-year nursing program AND three years of professional clinical nursing experience, including at least one year as described below Acceptable areas include nursing, health care administration, public administration in a health care area, public health, community mental health, community mental health counseling, psychology, sociology, biology, social work or human services. The 30 credits may include up to two statistics courses. Courses/degrees in physical education, recreation therapy, English, history, math, anthropology, languages, etc. are NOT acceptable. One year of experience in providing direct services to individuals diagnosed with mental illness, mental retardation, developmental disabilities, or alcohol or substance abuse in a COMMUNITY SETTING, e.g., a halfway house, visiting nurse, or a community clinic. SUBSTITUTION:
A master's degree in community mental health may be substituted for this experience only.Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
